PHS Report Improvements

We’ve made some improvements to the PHS report. Mostly cosmetic and aimed to keep the vertical size down. We also added a note to the footer signifying that the candidate has or has not certified the input (a box they have to check each time they submit the PHS). If the Candidate has altered the PHS in any way, this value gets set back to “not certified”. So this is a signal to you that the Candidate has not certified (or submitted) their current PHS as you see it.
